Item 1.01 Entry into a Material Definitive Agreement.
On September 10, 2009, Biolase Technology, Inc. (the "Company") entered into an
Amendment to License and Distribution Agreement (the "Amendment") amending the
License and Distribution Agreement, dated as of August 8, 2006, by and between
the Company and Henry Schein, Inc. ("Henry Schein"), as amended. Pursuant to the
Amendment, the Company and Henry Schein agreed to certain customer warranties in
respect of the Company's products sold to Henry Schein.
